      <image:caption>A discectomy is usually indicated when someone has a bulging, herniated, or ruptured disc that has failed conservative measures such as physical therapy. During a discectomy the part of the disc called the nucleus pulposus that has escaped the area it is supposed to be contained in, is removed. Usually a piece of the bone surrounding the area is removed as well, called a laminectomy. After the surgery, the most important this is protecting the surgery because it is actually easier to get another disc bulge, in the same area, after the surgery. In order to protect the disc bulge, you need to have great posture, avoid deep and lengthy forward bending (flexion), and use a lumbar roll while sitting. There are also some specific exercises that can help to protect the surgery, but those need to be prescribed specifically for your situation and monitored carefully.       <image:title>ACL Repair - ACL Repair</image:title>
      <image:caption>The anterior cruciate ligament is a vital ligament to knee stability when starting and stopping with running, cutting, and jumping/landing. It keeps the tibia (long bone bone below the knee) from sliding forward too far. When the ACL tears and the tibia slides forward other structures can be damaged such as the meniscus, MCL, cartilage, blood vessels, and joint capsule. Obviously with all the damage that can occur it is important to identify the issues ASAP. That is why you see the trainers and doctors rush out to athletes with knee injuries. In order to repair the ACL and re-stabilize the knee, the surgeon takes a portion of one of two tendons and threads it from the femur (long bone above the knee) into the tibia and anchors it down to both bones. While in surgery the doctor will also repair any other damaged structures leading the rehab to change due to healing time. There are 4 stages of rehab/recovery from an ACL repair: The protection phase involves immobilizing or limiting the mobility of the knee to allow the repair to stabilize and heal. Rehab involves swelling management, muscle activation, education, monitoring the wounds, moving within the protocol, and pain management. The mobility phase is to return the knee to normal motion while not stressing it too much. In some cases, the knee can be stiff due to the patient trying to protect it or scar tissue. If there is too much protection having a great therapist is vital to help over fears. The strengthening phase begins once the knee has healed and stabilized. Then, we can begin adding forces to strengthen the knee. There is an implementation of strengthening earlier for the hip and quad muscles to reduce muscle wasting. This phase is more functional strengthening such as leg press/mini squats etc. Return to play is the last phase, and it involves activities geared toward what the patient’s goal is such as sports. This is the fun phase for both patient and therapist. It is also one of the more important phases to reduce the risk of re-injury. How long it takes to return to sport is different with each person, but it can be discussed during your first visit.       <image:caption>The knee is a highly stable joint with many ligaments for support. However, the knee is also a tight joint especially during a dysfunctional or painful season. You may notice that your knee has begun swelling very easily, this is due to the tightness of the joint combined with being a joint that is in a dependent position (below the heart) for the vast majority of the day. Knee pain can come from a variety of sources including: Ligamentous Tears, knee sprains, and Osgood-Schlatter’s Disease to name a few. The goal of treatment is to restore mobility, abolish pain, and prepare the knee for the amount of force that you are going to put it through. In the vast majority of cases, the knee can be returned to normal without having to go through surgery (see Knee Arthroplasty).       <image:title>SLAP Tear Repair</image:title>
      <image:caption>SLAP stands for Superior Labrum Anterior Posterior. This tells you the area of the tear in your labrum, then the number afterward represents the severity of the tear. The labrum is part of the socket that holds the humerus (upper arm bone) into the shoulder joint. Labrum tears are common in older adults, and don’t always require a repair. For those that the surgery is indicated for, the surgery is performed arthroscopically and anchors are placed into the bone to hold the labrum in place. The rehabilitation process for a SLAP tear is lengthy and goes through several phases. You will begin with a protective phases where you are focused on protecting the surgery and carefully regaining range of motion (ROM). Then, you go through a recovery phase where the ROM is recovered at a faster rate and you begin some light strengthening. Next, is the strengthening phase where you greatly increase the strength of your shoulder and getting any last ROM you need to return to normal. Finally, is a return to sport/activity phase where you get to participate in much more advanced strengthening and recalibration of your muscles and brain to learn how to control your shoulder again. The key a successful rehabilitation process is a combination of following your home exercise program, and a physical therapist that will listen and get you prepared to get back to your desired activity.</image:caption>

Many people fail to realize their jaw is slowly stiffening year after year and making it more difficult to eat and speak. You are probably thinking what can they do for a joint that involves my mouth. In most cases, TMJ issues originate from the neck. When you think about it, as our head goes forward with poor posture (yes you have poor posture) our jaw is pulled down and forward leading to stress on the joint impacting it’s mobility. Fixing posture is a good place to start, but it won’t the the fix in most cases. Regaining the mobility in your neck and jaw is what improves the noise and locking that you may be experiencing.</image:caption>
